Patient Enrollment for New Patients and Annual Reenrollment
If you need healthcare, the Community Free Clinic and its partners may be able to help.
We are able to provide free healthcare and pharmacy services to adults in Cabarrus County who are uninsured.
WHO QUALIFIES?
To be eligible you must meet these requirements:
- Be a resident of Cabarrus County
- Have a household income at or below 300% of the federal poverty line
- Have no private health insurance, and do not qualify for Medicare/Medicaid or VA benefits
We now accept children into HealthNet Cabarrus. Pediatric services will be with area pediatricians.
How to Apply?
The Community Free Clinic is transitioning away from paper applications. Below is the enrollment application to complete online. If you don’t have an email address, you can fill in and print the application to return to us. You also can visit us at the Community Free Clinic at 528A Lake Concord Road, NE, Concord, NC, 28025. We have enrollment specialists who can help you. Enrollment specialists are available 8:30 am – 5 pm Monday – Thursday. Hablamos español.
If you come in person, please bring a digital/online copy of your support documents. To apply online, you will need to attach support documents to the application to complete it.
What information do I need to provide?
Please complete the Patient and Clinic Agreement application, which includes proof of address and proof of income. Once your Enrollment Specialist reviews the application, they may ask for additional documents.
Please also complete the MedAssist application. MedAssist allows us to provide long-term medications your provider prescribes to you for free. Your MedAssist application will be submitted directly to MedAssist.
1. Patient Enrollment Application
The Community Free Clinic has an initial enrollment to become a patient, and a reenrollment every year from the date of completion.
With this enrollment you will receive a HealthNet Cabarrus Pink Card document to take with you when you visit a clinic or hospital. The Pink Card is NOT insurance, and the Community Free Clinic is not an insurer. The Pink Card lets other hospitals and clinics know that you are a patient enrolled in a community-wide effort to provide healthcare and medications, called HealthNet Cabarrus. The Community Free Clinic is the enrollment location, with other participating locations throughout Cabarrus County.
Be prepared to attach documents for proof of residency and proof of income.
Accepted formats: .jpg, .png, .doc, .pdf
Documents that prove residency:
- Valid North Carolina driver license with current address
- Government issued ID with current address
- Government issued letter, utility bill or medical bill dated within the last 30 days
- Valid and current lease agreement
Documents that prove income:
- Wages – Pay stubs for the last 30 days or income verification form completed by employer. If your pay varies, please provide stubs from several months.
- Self-Employed
- Unemployment Benefits – Notice of benefits statement
- Supplement Social Security Income (SSSI) – Current year benefit letter
- Disability – Current year benefit letter
- Pension/Retirement – Current year benefit letter
Once you are enrolled, you will reenroll annually. We will send reminder calls and emails that your enrollment is up for renewal. If you choose not to reenroll, you will not be able to get an appointment or prescription for medications.
You can fill out your application online, or you can fill in and print your application to mail and bring to us.

2. MedAssist Application
Applying to MedAssist is an important step to receive medications for free. The Community Free Clinic is a paid Partner in Hope to MedAssist to receive this service for you. Filling out this additional application fully means from your first appointment, if you need a prescription, your provider will send it to MedAssist and the medication will come to your home for free. If MedAssist is unable to help you, the Clinic will work with the medicine’s manufacturer to help you get it for free.
If you choose not to fill out this application, you will have to pay for your medication. The Clinic will send your prescription to a pharmacy that you choose. If you don’t have a pharmacy that you like, the Clinic can recommend a pharmacy that has that particular medication at a low cost.
MedAssist requires this separate application and renews every 11 months (the Clinic’s application renewal is every 12 months). MedAssist will remind you when your application is up for renewal.
You will need to upload documents to the MedAssist Application.
Once you complete the application at the link above, an enrollment specialist will contact you one business day later (Monday – Thursday are our business days) to confirm information and ask questions. Once you are enrolled at the Community Free Clinic and have a HealthNet Cabarrus Pink Card, our Clinic staff will set your first appointment.
